Laboratory diagnostics of male infertility have largely focused on morphological assessments. However, semen analysis (spermogram) is often subject to subjective interpretation and significant variability. In contrast, biochemical tests offer greater stability but are not well-suited for direct analysis of sperm cells. The multifactorial nature of male infertility — influenced by social, environmental, and demographic factors — further complicates the diagnostic process. As a result, there is a growing need for novel approaches to detect spermatogenic dysfunctions, such as reduced sperm concentration, impaired motility, or increased morphological abnormalities. Current research efforts are directed toward the development and validation of biomarker-based diagnostic tools. The identification of relevant biomarkers, along with investigations into genetic and epigenetic alterations, the role of the microbiome, and the integration of automated analysis systems, holds promise for enhancing diagnostic precision. These innovative strategies may lead to more accurate diagnoses and enable the implementation of personalized treatment plans.
