Skin fibroblasts are highly active mesenchymal cells act as key modulators of cutaneous structure, homeostasis, and healing. Low-level laser therapy has been demonstrated to expedite wound healing processes, increase collagen synthesis, and stimulate fibroblast proliferation in skin tissue. Exosomes are endosomal compartment-derived, nanoscale extracellular vesicles released by all cell types.The study aimed to assist the effect of Exosome-(hASCs) and   Low Level Laser Therapy on fibroblast cells proliferation in vitro study. HDFCs were cultured and treated with different concentrations of exosome (1,5,10,15 and ,20 μg/ml). After that, cells were exposed to a continuous mode, 830 nm, 10 mW gallium–aluminum–arsenide (GaAlAs) laser for one minute.cells Proliferation and metabolic activity were evaluated using MTTand ATP assays at (24h and 48h). A significant proliferations at  ( p<0.05) appeared in groups treated with 15and 20  μg/ml  (1.30±0.07and 1.40 ±0.05)and (1.44 ± 0.01 and 1.60 ± 0.03) in compared with control group(0.80 ± 0.01and 0.82  ± 0.03)  after  24h ,48h respectively. the proliferation was (1.10 ± 0.03) for Laser group while Laser +Exosome recorded a significant effects at p<0.05 on cells proliferation especially in Laser+ 20 μg/ml group  (2.51± 0.05)  in compared with control group 0.65±0.01.The results  illustrate that ATP level increased in all groups at increasing Exosome concentrations ( 11688.56, 11966.67, 12384.7, 12834.65, 13674.67,and 14510.33) in comparing with control group (8675.667). Exosomes - (hADCs) showed a synergistic enhancement of fibroblast proliferation when combined with low-level laser therapy .Optimal stimulation was achieved by subjecting cells to low-level laser therapy for one minute at 10 mW (3.78 J/cm²) with 20 μg/mL exosomes.
